This document outlines the community health needs assessment of the service area of National Jewish Health, a specialty care hospital serving the city of Denver and surrounding communities. The eight health needs identified from this assessment, including obesity, respiratory disease, and tobacco use and cessation, are explored and a plan for addressing these needs is also provided.
