This report details nine case studies of innovative programs striving to address the gap between the health and housing sectors.
Healthcare professionals recognize their patient’s need and plan members for affordable housing, but may not know how to enable the creation of housing for those that need it. Similarly, housing developers are aware that providing safe, affordable and high quality housing to high-need individuals would be valuable, but may not know how to form a partnership with the appropriate health agencies.
This report hopes to assist entities to move from the pilot response stage toward a more systemic response.
